Shirley Ann Kozak

Ontario – Shirley passed away on Tuesday, March 1, 2022.  She was predeceased by her father, Charles Campbell; sister, Diane Lynch; brother, Robert Campbell; niece, Donna Marie.

Shirley is survived by her husband of 56 years, Gerald A. Kozak; children, Lisa (Dave) Warfle and Denise (Timothy Taylor) Kozak; grandchildren, Gerald A. Kozak II, Nichole Kozak, Brandon and Chelsie Graffrath; mother, June Campbell; brother, Charles (Ellie) Campbell; several nieces and nephews.

Family and friends may gather for visitation from 1-4 PM on Sunday (March 6) at Murphy Funeral & Cremation Chapels, 1961 Ridge Rd., Ontario, NY. A Mass of Christian Burial for Shirley will be offered on Monday (March 7), 10:30 AM at St. Mary's of the Lake Church, 5823 Walworth Rd., Ontario, NY.  Shirley will be laid to rest in Calvary Cemetery.

In lieu of flowers, contributions in memory of Shirley may be directed to National Kidney Foundation, 1344 University Ave. #270, Rochester, NY 14607.
